Save time at your first appointment by completing your forms in advance.
You can download, print, and fill them out before your visit — then bring them with you or fax them to our office.

If you prefer, our Patient Service Representatives can provide copies for you to complete during check-in.
Insurance Information
As a courtesy, we bill most private insurance plans.
Miller Dermatology is a
preferred provider with:
- Blue Cross Blue Shield
- Aetna
- EBMS
- Meritain
- Medicare
- Other insurances through the Multiplan network
We are not preferred providers with Medicaid, Tricare, VA, or Campus.
If you’re unsure about your coverage, please call (907) 276-1315 or contact your insurance company for confirmation.
Financing options are also available for cosmetic treatments.
Preparing for Your Visit
To ensure your appointment goes smoothly, please:
Arrive 10–15 minutes early for check-in.
Bring your ID, insurance card, and completed forms.
Bring a list of any medications or skin care products you currently use.
Inform us of any allergies or prior reactions to medications.
Our team will handle the rest — and make sure you feel comfortable and informed every step of the way.

Do you accept walk-ins?
We see patients by appointment only to ensure each visit receives full attention and care.
Do I need a referral?
Most patients can schedule directly, though some insurance plans require a referral. Our staff can help you verify requirements.
Do you offer telehealth visits?
Telehealth appointments may be available for follow-ups or medication reviews. Call our office for details.
